Referrals

Making a referral to the Hope Team does not mean an individual is eligible. The eligibility process can take several months so please continue with current treatment and bridge services until eligibility can be determined.

Referral
  • Referrals can be made by anyone (Individual, Family member, or Provider) by calling 412-647-4321, emailing at hopeteam@upmc.edu, OR by completing our Online Screener and requesting to be contacted.
  • The referred individual will be contacted within 2 business days to schedule a phone screen.
Phone Screen
  • The phone screen will take 30-60 minutes to complete and will determine whether an intake evaluation is recommended.
  • Person screened will be contacted within a week to schedule an intake evaluation (if eligible) or to provide recommendations on more appropriate services (if not eligible).
Intake Evaluation
  • Hope Team staff will contact the individual to schedule an intake evaluation within one week of an eligible phone screen.
  • Due to demand, intake evaluations may be scheduled 1+ months away.
  • Intake evaluations can be provided in person or via telehealth, take about 2.5 hours, and determine whether an individual is eligible for Hope Team Services.
  • People will be contacted by the Hope Team within a week of the intake with feedback so that they can begin the enrollment process (if eligible) or to provide recommendations for more appropriate services (if not eligible).
  • If the referral was made by a community provider and the person being assessed agrees, the provider will be notified of the outcome of the intake evaluation after the family/ individual has received feedback.
Enrollment
  • The individual will complete additional assessments and reports; this visit will take ~ 1.5 hours
  • The individual and their family will determine which HOPE TEAM services they would like to utilize and will be scheduled for those services.
